UponFutures Support Services supports participants all over Australia including Victoria, New South Wales, South Australia, Queensland and Tasmania.

Support Coordination
Our wonderful team of Support Coordinators at UponFutures Support Services have the capacity to assist participants within our community to be able to implement and navigate your NDIS plan.

We can help you to access appropriate services and providers in areas such as Occupational Therapy, NDIS Supports, STA, SIL, MTA and SDA Support, Community Supports and support in your own home.Plan implementation -
Our Support Coordinators will support you in understanding your NDIS plan, Advocate with and on behalf on you and your needs, ensuring that you are receiving effective and adequate support through your plan. If you are not satisfied with your NDIS plan, our Support Coordinators will support you with your plan review and helping you get the appropriate funding and support.Solve problems in your NDIS plan -
Unfortunately, issues arise at times with NDIS plans. If you are not satisfied with your NDIS plan, our Support Coordinators will support you with your plan review and helping you get the appropriate funding and support and aim to find a solution.Capacity Building -
We will work with you and your Supports to develop skills, knowledge and abilities to reach and achieve your goals. We focus on empowering our participants to take control and increase their independence over time.Monitor progression -

In Home Support
UponFutures Support Services provides In Home Support to Participants with a Disability. This allows us to support you with daily tasks and activities within your home whilst helping you live more independently. We ensure to provide support tailored to your needs.

Specialised Disability Accommodation
UponFutures Support Services provides SDA (Specilist Disability Support) to Participants in need of housing designed to meet the needs of participants high functional impairments or with high requirements of support.
Our Specialist Disability Accommodation are built and modified to accommodate Participants with high physical, sensory or cognitive support needs. These supports include wheelchair accessible housing, hoists, wide doorways, modified bathrooms and other customisations to ensure a safe and comfortable living environment.
Our SDA is also intended for individuals who require significant daily assistance, including complex Physical Disabilities, Neurological Disabilities, Participants requiring significant medical care and personal care.
Supported Independent Living
UponFutures Support Services offers SIL (Supported Independent Living) to Participants needing support to live independently in a shared living space. Our Supported Independent Living residences are designed to help participants with disabilities to live as dependently as possible while receiving the necessary support to live their daily lives and manage daily living activities. Participants will have Support Staff available to assist them with Personal Care, Daily Tasks and other aspects of Daily living.
Short Term Accommodation
Short Term Accommodation with UponFutures Support Services is a service that allows for temporary care and support to participants with a Disability. This allows participants to take a break temporarily, giving people with a disability the opportunity to develop independence, try new experiences and continue being in a safe and stable environment.
During a Participants stay, UponFutures Support Services will continue to provide support with daily living support, community access, transport and activities. Participants will be supported in achieving specific goals, trying out new environments or engaging in activities outside of a participants routine. STA can also serve as an opportunity for Participants to engage with others, take part in recreational activities, learn new skills and abilities.
Community Access Support
UponFutures Support Services provides Community Access to our participants to allow them to participate and engage in a community setting. We focus on empowering individuals to take part in social, recreational, educational and employment activities outside of a persons home.

Transport Assistance
We aim to assist individuals with a disability to build connections, gain new experiences, be active within their local communities, enhance our participants independence and social inclusion.
Day Programs
Our Day Programs allow for a structured environment where participants can learn new skills, build new and existing relationships, engage within the community and enjoy new group based activities. Depending on your needs and interests, we will tailor our day programs to suit your needs.
Holiday Programs
Our holiday program at UponFutures Support Services offers participants to have a fun and enjoyable time which includes community - based outings and activities, social engagement and recreational opportunities. This can include activities such as
